The application came to us in multiple parts. We thought the original 10 pages or so was the full application, but in fact, that was only the first part. In total, we probably filled out about 20 pages of paperwork although some of it turned out to be repetitive information.

Each states application looks different, but heres the questions you can expect:
- General information about the household with questions about contact information, education and income.
- Your familys history with drugs and alcohol and psychiatric treatments not just your households history but those in your extended family as well.
- Marriage and relationship history about divorce, co-habitation, and to some degree, your sexual history.
- Whether child neglect or abuse allegations have ever been filed against you or a family member.
- Financial worksheet that details what you are worth and how much you spend. This includes all monthly expenses and how much is in your various bank accounts 401(k), investments, and what your properties are worth. It also includes questions regarding debt on mortgages, loans and credit cards.
In addition, along with the application youll submit:
- Health reports. Every member of the household needs to see a doctor who will fill out a health report detailing if youre all got the required immunizations and are in good health.
- Fingerprints. Each member of the household above 18 will have to participate by submitting to an extensive background check that includes finger printing that gets cross-checked in the state database as well as the FBI database.
- Copies of all birth certificates, marriage certificates, Social Security cards, and drivers licenses.
- Copies of recent pay stubs and tax returns.
- Releases so that previous counselors or social services workers can be interviewed.
- Autobiographical questions about your relationship with your immediate + extended family, details about childhood experiences + traumas, and reasons for deciding to go the foster Adoption route.
-Several references who have known you for several years and can speak about your character including a reference from your employer.

Keep in mind, each state has its own rules so the info I share is specific to adopting in Wisconsin… although there is a lot of cross-over in most states.
Where are we currently at in the process? As of this video’s posting date we’re just finalizing our home study and will be eligible for placement next month.

Let us start by saying THANK YOU in advance for any and all support in any form for our family.
Harvey and I started dating almost 9 years ago! Wow, time flies when your in love and happy.
Harvey was previously married and has two wonderful girls, Ariel 16 and Sabrina 13, Harvey thought he was going to be married forever and his desicion was to not have more than his two girls. Harvey was pretty young when deciding this and it was a hard decision but it was his and he did indeed go under the knife and now cannot have any children. What Harvey did not expect was to no longer be married to his first wife and to get remarried.
Harvey and I were married almost 5 years ago and we do want a child. We would love to bring home a little one, boy/girl, doesnt matter as long as he or she is healthy! We have looked into every which way to get his surgery reversed, invitro etc. It really doesnt look like any of them are an option for us.
We started to look at Adoption, we had all the questions in our heads about what to do, how it would affect everyone etc. But, after researching and seeing how many children need a happy, healthy home to go to, it would be selfish of us not to go this way.

We’re Dave and Jen and we are hoping to become first-time parents by adopting a baby through Open Adoption. We live in Orlando, FL, and have been married for almost 12 years. We live in a quiet, kid friendly neighborhood with excellent schools. We enjoy watching shuttle launches from our backyard and playing at Disney on the weekends. Jen works in communications. Dave is a carpenter. Dave’s family was built through adoption.
This video is our prospective adoptive parent video profile and shows a glimpse into our life together.
We are home study approved and working through A Chosen Child agency in Orlando, Florida.
